** Child Support Obligation Income Statement/Affidavit (both parties)(CS-41)
** Child Support Guideline Form (form CS-42)
** Child Support Guideline Notice of Compliance (form CS-43)
** Withholding Order Payment of Child Support (form #CS-34 Instructions for Employer )
** Children Cope with Divorce Seminar Certificate (both parties)

Service fee for an immediate Withholding Order is $12.50 by sheriff or $5.71 by certified mail.

FILING FEE

Court Cost for filing of Divorce is $151.00, payable only by cash, money order or cashier's check. Filing Fee's are non-refundable.
